Recipients of the Care Planning - Fundamental Skills can create coordinated care plans for clients with or at risk of chronic diseases and support them in achieving health goals. They have case conceptualization skills, or the ability to consider the biological, psychological, and social contexts of clients, to develop appropriate coordinated care plans at the fundamental level. They have interpersonal skills needed to coordinate shared decision-making with clients and other care providers.
