Physician member of the American Society of Addiction Medicine for at least two (2) consecutive years
Currently certified in addiction medicine by the American Board of Addiction Medicine (ABAM), the American Board of Preventive Medicine (ABPM), the American Osteopathic Association (AOA), the American Board of Psychiatry and Neurology (ABPN) in Addiction Psychiatry, or the International Society of Addiction Medicine (ISAM)
