Organizations that achieve CHAP Accreditation may have also been deemed to meet the Medicare Conditions of Participation and is certified as a Medicare provider. Based on the findings of the site visit conducted, at the location and service referenced above, the organization is found to be in compliance with the CHAP Standards of Excellence. The CHAP Board of Review has granted Accreditation for the term of three (3) years.
