Submit your application, accept the terms of the candidate handbook and submit payment.
Receive a passing score on the multiple-choice examination consisting of 140 questions.
Upon passing the examination, certified professionals are entitled to use the designation Certified Professional in Healthcare Quality® and acronym “CPHQ”.
